With open enrollment starting in October, you can finally choose the Omnipod® System through
Medicare Part D.

Coverage through the Part D pharmacy benefit means:
- Coverage for patients with both type 1 and type 2 diabetes
- Streamlined electronic fulfillment process
- Convenient access to supplies through pharmacy or mail order
- You’re no longer locked into a 5-year pump contract
If you’re not covered on an eligible Medicare plan, you can still get an initial coverage determination in 72 hours or less. Here’s all you need to do:
You or your physician
Submit coverage determination request forms to the plan sponsor.
To get the forms, call 1.877.939.4384 or you can visit the CMS website to obtain the necessary forms.
Your Part D Plan Sponsor
Determines coverage or if additional information is required. In some cases, you may need to follow an appeals process.
If your coverage is approved, you may be eligible to receive reimbursement for the Omnipod® System.
*At start up, Pod and PDM must be adjacent and touching. During normal operations, Pod must be within 5 feet (1.5 meters) of the PDM.
*The Omnipod® System has a waterproof IPX8 rating for up to 25 feet for 60 minutes; the Personal Diabetes Manager is not waterproof.
